*** For LASER HAIR REMOVAL / IPL / SPIDER VEIN / SUNSPOT REMOVAL Clients:

- I am over 18 years of age.

- I have not taken antibiotics, iron supplements, or used RETIN A in the last 14 days.

- I have not been exposed to the sun, tanning beds or had a spray tan or used gradual tanning lotions, sprays or drops within the last 2 weeks.

- I have not taken Accutane or immunosuppressants in the last 6 months.

- I am currently not pregnant.  

- Come in without makeup, numbing creams, lotions, and the area(s) to be treated clean shaven.

- I have not plucked, waxed or used depilatories within the last 4 weeks on the area(s) to be treated.

- You must shave the area(s) being treated within 24 hours of your Laser Hair Removal appointment.

FAILURE TO FOLLOW THE ABOVE INSTRUCTIONS MAY RESULT IN NO TREATMENT AND HAVING TO RESCHEDULE YOUR APPOINTMENT.

*** For SKIN SERVICE Clients:

CONSULTATIONS MUST BE BOOKED WITH THE SAME PROVIDER FOR TREATMENT AS WELL. PLEASE DO NOT BOOK WITH 2 DIFFERENT ESTHETICIANS FOR CLOSULTATION AND SERVICE.

- Come without any makeup on, and with the treatment area clean.

- I have not been exposed to the sun, tanning beds or had a spray tan or used gradual tanning lotions, sprays or drops within the last 2 weeks.

- I have not used any resurfacing skin care products or medications like retinoids and exfoliating acids for the last 3 days.

- Do not come if you have an active infection on the area(s) being treated.

- I have not had Botox, Dysport or any neurotoxin within 2 weeks and or filler within 4 weeks.

- Each treatment has its own contraindications and pre/aftercare instructions. Please follow your AP Estheticians instructions.

FAILURE TO FOLLOW THE ABOVE INSTRUCTIONS MAY RESULT IN NO TREATMENT AND HAVING TO RESCHEDULE YOUR APPOINTMENT.
